Usage: Selective aldosterone receptor antagonist (SARA), structurally similar to Spiranolactone. Eplerenone is used alone or in combination with other medications to treat high blood pressure. Eplerenone is in a class of medications called mineralocorticoid receptor antagonists. It works by blocking the action of aldosterone, a natural substance in the body that raises blood pressure.
Eplerenone Description:
Eplerenone (INN) /plrnon/ is an aldosterone antagonist used as an adjunct in the management of chronic heart failure. It is similar to the diuretic spironolactone, though it is much more selective for the mineralocorticoid receptor in comparison (i.e., does not possess any antiandrogen, progestogen, or estrogenic effects), and is specifically marketed for reducing cardiovascular risk in patients following myocardial infarction. It was marketed byPfizer under the trade name Inspra but is now only available as a generic in every country except the United Kingdom. Eplerenone is a potassium-sparing diuretic, meaning that it helps the body get rid of water but still keep potassium.
Do NOT use eplerenone if:
1. you are allergic to any ingredient in eplerenone
2. you have severe kidney problems or high blood potassium levels
3. you are taking eplerenone to treat high blood pressure, and you have type 2 diabetes with protein in your urine
4. you are taking clarithromycin, itraconazole, ketoconazole, nefazodone, nelfinavir, ritonavir, or troleandomycin
5. you are taking eplerenone to treat high blood pressure, and you are taking potassium supplements or potassium-sparing diuretics
Important safety information:
1. Eplerenone may cause dizziness. This effect may be worse if you take it with alcohol or certain medicines. Use eplerenone with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it.
2. It may take up to 4 weeks to see the full benefit of eplerenone on your blood pressure. Do not stop taking eplerenone without checking with your doctor.
3. Do not exceed the recommended dose of eplerenone. Doing so will not improve your condition faster and may lead to side effects.
4. Do not use a salt substitute or a potassium supplement without checking with your doctor.
Tell your doctor or dentist that you take eplerenone before you receive any medical or dental care, emergency care, or surgery.
5. Patients who take medicine for high blood pressure often feel tired or rundown for a few weeks after starting treatment. Be sure to take your medicine even if you may not feel quite "normal." Tell your doctor if you develop any new symptoms.
6. Lab tests, including blood potassium levels, blood pressure measurements, and kidney function, may be performed while you use eplerenone. These tests may be used to monitor your condition or check for side effects. Be sure to keep all doctor and lab appointments.
7. Use eplerenone with caution in the ELDERLY; they may be more sensitive to its effects, especially high blood potassium levels.
8. Eplerenone should be used with extreme caution in CHILDREN younger than 4 years old; safety and effectiveness in these children have not been confirmed.
9. PREGNANCY and BREAST-FEEDING: If you become pregnant, contact your doctor. You will need to discuss the benefits and risks of using eplerenone while you are pregnant. It is not known if this medicine is found in breast milk. Do not breast-feed while taking eplerenone.
